They have filled out the forms so that you can easily compare them to other surgeons on the list below. The list of surgeons who have paid a fee to have their profile available to prospective patients for comparison is below.
You may receive the profiles by ordering them using the directions at the bottom of the page. Individual profiles will not be made available, you will receive all of them in order for you to make comparisons and choose the surgeon that is best for your situation.
The fee to order the entire Doctors' Profiles packet including all surgeons listed below is $5.95. This is to offset the cost of copying and includes shipping and handling anywhere in the continental United States. The Doctor Profile packet will be mailed to you via Priority Mail.
